DD Form 1172-2 Application for Identification Card/DEERS Enrollment (76k, PDF) Instructions for DD Form 1172-2 (152k, PDF)	This form is used to enroll in DEERS and is most often used to enroll dependents of servicemembers.
DD Form 2841 DoD Public Key Infrastructure (PKI) Certificate of Acceptance and Acknowledgment of Responsibilities (98k, PDF) Instructions for DD Form 2841 (8k, PDF)	This form is the Public Key Infrastructure (PKI) certificate of authorization and acknowledgement of responsibilities for the registration official.

RAPIDS Self-Service (RSS)	The RAPIDS Self-Service (RSS) Web application is used to update information stored on a user's Common Access Card (CAC).
Verifying Officials Information System (VOIS)	Information for professionals tasked with verifying identification.
