DD Form 1172-2 Application for Identification Card/DEERS Enrollment (76k, PDF) DD Form 1172-2 Instructions (152k, PDF)	This form is used to enroll in DEERS and is most often used to enroll dependents of servicemembers.

Common Access Card User Maintenance Portal (UMP)	The User Maintenance Portal (UMP) Web application is used to update information stored on a user’s Common Access Card (CAC).
Verifying Officials Information System (VOIS)	Information for professionals tasked with verifying identification.
